And then there are the eyes. The eyes make it worse because while the mouth is full of tension and struggle, the eyes go completely blank in a way that does not match the rest of the face. That hollowness makes the whole expression look inhuman, as if two separate parts of the face are disagreeing with each other: the top half is dead, while the bottom half is panicking. This creates a strange disconnection—a broken expression that feels eerie, uncanny, and unsettling to look at. Now, you may be wondering why I am describing this look to you. Why do narcissists have a face like that? Let’s find out in today’s episode.
Why Narcissists Exhibit the Fish Mouth
When a narcissist makes that fish mouth, they signal that danger is seconds away because this expression is the physical manifestation of narcissistic injury. When you see that mouth tighten and push forward, it means you have done the one thing a narcissist cannot survive: you have pierced their reality. You denied them something they felt entitled to or exposed the truth they were desperately trying to bury. That little push of the lips comes out when their ego takes a direct hit. When that mouth pushes out, they are drowning in a level of internal shame that their psyche is not built to handle.
Becausethe narcissist cannot tolerate shame—not even for a fraction of a second—their brain has to convert it into something they can use, which is hatred. The hatred for you becomes the only way they can escape the emotional collapse happening inside them. That swollen look is the pressure building; it is the quiet before the eruption. In this moment, they are calculating whether to smear you, explode at you, or withdraw into an icy silence, punishing you in a way that will make you regret ever seeing through them.
